1. Agreement

By using FormStack's services, you agree to these Terms of Service. If you use FormStack on behalf of an organization, you represent that you have authority to bind that organization.

2. Service Description

FormStack provides a web-based form building platform that enables users to create, customize, publish, and analyze online forms, surveys, and quizzes. The service includes the form builder, response collection, analytics dashboard, and integrations.

3. Your Account

  • Provide accurate registration information
  • Keep credentials confidential
  • Notify us of unauthorized access
  • You are responsible for all activity under your account

4. Content Ownership

You retain all rights to the forms you create and the response data collected. You grant us a limited license to host and display your forms, and to process response data solely to provide the service.

5. Acceptable Use

You agree not to use FormStack to:

  • Collect data you're not authorized to collect
  • Send spam or unsolicited communications
  • Create phishing or deceptive forms
  • Violate any applicable laws or regulations
  • Interfere with the service's operation
  • Reverse engineer or copy the platform

6. Payments

Paid plans are billed in advance on a monthly or annual basis. All fees are non-refundable except under our 30-day money-back guarantee. We may change pricing with 30 days' notice.

7. Uptime & Support

We target 99.9% uptime for all paid plans. Support is available via email, chat, and phone depending on your plan level. We provide reasonable efforts to resolve issues promptly.

8. Limitation of Liability

TO THE MAXIMUM EXTENT PERMITTED BY LAW, FORMSTACK SHALL NOT BE LIABLE FOR INDIRECT, INCIDENTAL, SPECIAL, OR CONSEQUENTIAL DAMAGES. OUR TOTAL LIABILITY SHALL NOT EXCEED THE FEES PAID IN THE 12 MONTHS PRIOR TO THE CLAIM.

9. Termination

Either party may terminate at any time. Upon termination, you can export your data for 30 days. After that period, data is permanently deleted. We may suspend accounts that violate these terms.

10. Changes

We may update these terms with 30 days' notice. Continued use after changes take effect constitutes acceptance.
