By using MediCare Plus services, you agree to these terms. Please read them carefully.
1. Patient Responsibilities
As a patient of MediCare Plus, you agree to:
- Provide accurate and complete medical history and information
- Follow treatment plans and medical advice provided by our healthcare professionals
- Arrive on time for scheduled appointments or provide 24-hour notice for cancellations
- Pay for services rendered according to your insurance coverage or payment arrangement
- Treat staff and other patients with respect and courtesy
2. Appointment Policy
- Scheduling: Appointments can be made by phone, online, or in person
- Cancellations: Please provide at least 24 hours notice to avoid cancellation fees
- No-Shows: Patients who miss appointments without notice may be charged a no-show fee
- Late Arrivals: Patients arriving more than 15 minutes late may need to reschedule
3. Payment and Billing
- Payment is due at the time of service unless prior arrangements have been made
- We will bill your insurance company as a courtesy, but you are ultimately responsible for payment
- Co-pays and deductibles must be paid at the time of visit
- Outstanding balances are subject to late fees and may be sent to collections
- Payment plans are available for qualifying patients
4. Medical Records
- You have the right to access your medical records
- Record requests must be made in writing and may require a processing fee
- We maintain records according to federal and state regulations
- Records may be released to third parties only with your written authorization or as required by law
5. Prescription Policy
- Prescription refills require 24-48 hours notice
- Some medications may require an office visit before refill
- We do not prescribe controlled substances for initial visits without proper documentation
- Lost or stolen prescriptions will not be replaced without a police report
6. Patient Portal
- You are responsible for maintaining the confidentiality of your login credentials
- Do not share your portal access with others
- Report any unauthorized access immediately
- Portal messages are not for urgent medical matters—call 911 for emergencies
7. Limitation of Liability
MediCare Plus provides healthcare services to the best of our ability but cannot guarantee specific outcomes. We are not liable for:
- Outcomes resulting from failure to follow medical advice
- Adverse reactions to medications not previously known
- Complications from conditions not disclosed by the patient
- Services provided by third-party healthcare providers
8. Termination of Care
We reserve the right to terminate the physician-patient relationship for:
- Non-compliance with treatment plans
- Abusive or threatening behavior toward staff
- Repeated missed appointments without notification
- Non-payment of outstanding balances
- Drug-seeking behavior or misuse of prescribed medications
In such cases, we will provide 30 days notice and assist with finding alternative care when appropriate.
9. Changes to Terms
We may update these terms from time to time. Continued use of our services after changes constitutes acceptance of the new terms. Significant changes will be communicated to patients.
